Mrs. Catherine Ciocca and Mrs. Tracy Przybylowski are Devon Prep’s School Nurses. Mrs. Ciocca holds a BSN in Nursing from Gwynedd Mercy
A School Nurse is on duty throughout the school day to provide for the basic medical needs of students who become ill or injured during the school day.
ILLNESS
Parents are asked to use their best judgment when deciding if their son is well enough to attend school, except in the following circumstances, when the student MUST not be in school:
- Fever in the last 24 hours (100 degrees or above)
- Vomiting in the last 24 hours
- Diarrhea in the last 24 hours
- Unidentified rash
- Drainage from the eyes/eye
- Persistent cough
Immunization/Physical/Dental requirements required by the State of Pennsylvania
- All new students must bring in an updated immunization record and physical dated within one year upon enrollment.
- Any new student coming from outside the state of Pennsylvania also needs an updated dental record and physical dated within one year.
Middle School
- All 6th graders must turn in a full physical with immunizations dated within one year before the first day of school.
- All 7th graders must bring in a dental form dated within one year.
- All 7th graders must provide documentation before the first day of school of receiving FIRST MCV (Meningitis vaccine) and T-Dap. (diphtheria, pertussis, and tetanus vaccine)
High School
- All Juniors must turn in a full physical with immunizations dated within one year.
- All seniors must provide documentation before the first day of school of receiving SECOND MCV (Meningitis vaccines).
Any student that does not comply will be dismissed from school after 5 days from the 1st day of school.
PHYSICIAN NOTES
Parents must deliver a physician’s note to the nurse in the following circumstances:
- The student is required to use crutches or any device for ambulation. The physician’s note must be sent on the first day the crutches/device is to be used. The note must include directions on the use of crutches/devices.
- When a student is excused from physical education or sports by a physician. Another note will be needed when they are cleared to return to physical education or sports.
- The student is absent for 3 or more consecutive days due to illness/injury.
DISMISSAL FROM SCHOOL
Dismissal from school due to illness or injury must be approved by the nurse. The nurse must be informed by the student of the illness and evaluate. The nurse will then contact the parent. Parents who are picking up their son must report to the main office. The receptionist will notify the nurse of your arrival. Parents are asked to not go unescorted to the health room for reasons of comfort and confidentiality of the other students who may be there.
MEDICATION
- All medications (prescription and over the counter) to be given in school and on field trips (day or overnight) require a physician’s order AND parent/guardian consent (form can be found on Devon Prep website).
- The parent/guardian must bring medication to school. If the medication is a controlled substance, the nurse receiving the medication will document the quantity of the medication delivered. This documentation will include the date, time, amount of medication, and the signatures of both the parent/guardian and the nurse receiving the medication.
- All medications must be brought in their original packaging/pharmacy packaging. No loose medication will be accepted.
- All medication orders must be renewed at the beginning of each academic year.
- The nurse WILL NOT give the first dose of any new medication.
- No student may carry any medication in their backpack or their person unless it is a life-saving medication with a physician order.
- If a student requires a life-saving medication, such as an inhaler, epinephrine auto-injector and/or insulin, etc, he may ‘self carry’ with parent permission AND physician order (form can be found on Devon Prep website). The parent and MD will assess and determine if the child is responsible enough to carry AND administer in an emergency. The nurse and/or school administrator can revoke these privileges at any time with permission from the parent. The nurse must be notified if the emergency medication is used by the student.
- Benadryl, Epinephrine Auto-injectors, Narcan, inhalers, and aspirin (for an adult) may be given by teachers/faculty/chaperones as part of emergency treatment. 911 must be called after giving emergency treatment (see Tredyffrin Easttown School District Standing Orders located in the health office).
- All medications not used expired or discontinued will be returned to the parent/guardian. If the parent/guardian does not retrieve the medication by the end of the school year, the nurse and one witness will dispose of the medication and document the disposal.
FORMS AND PERMISSION
- An Emergency Medical Information Card must be filled out by a parent/guardian at the beginning of the school year. If there are changes in the student’s medical status, then the nurse will be notified of changes. (This will be provided to you in the summer before the school year.)
- The Field Trip Health and Medication Form is electronic and only needs to be filled out and submitted ONCE unless there is a change in health status. The form can be found on the Devon Prep website.
- Any student requiring Epinephrine must have the Food Allergy and Emergency Care Plan Form filled out by a physician. This form can be found on the Devon Prep website.
- Any student requiring ANY medication (prescription and over the counter) must fill out the Physician/Parent Medication Form found on the Devon Prep website.
FIELD TRIPS
- Students that do not have their Emergency Cards/Medication and Health Forms completed will NOT be able to attend a field trip.
- Students bringing non-life saving medication on a field trip/school activity may not carry the medication. If the nurse attends the field trip, she will hold the medication and the student will visit the nurse to take medication during prescribed times. If a nurse is not in attendance, the teacher/chaperone will hold the medication, but the student will self-administer the medication.
- If a nurse is NOT attending the field trip then the chaperone may NOT give standing orders (Tylenol, Ibuprofen, Tums) without calling parent/guardian and getting permission (see Tredyffrin Easttown School District Standing Orders located in the health office).
- All doctor’s orders and medication must be given to the nurse 2 weeks before the scheduled field trip.
- Parents must send only medication that is necessary and only the amount needed while on the field trip.

Dismissal from school due to illness must be done by the nurse. The nurse will contact a parent and a parent/guardian must sign out the student in the Main Office before he can be dismissed.
